Hi, I'm Yasmin. I am the Care Contracts Officer, holding strategic and operational responsibility for the coordination and oversight of referrals, comprehensive suitability assessments, engagement with new and existing commissioning boroughs, negotiation and monitoring of funding arrangements, and the effective management of discharge processes. My role ensures that all placements are appropriately matched, contractually robust, financially authorised, and aligned with statutory and organisational requirements from initial referral through to transition or discharge.
Previously, I served as Area Manager for Surrey, where I maintained full operational and strategic oversight of five residential homes supporting approximately 20 clients. In this capacity, I was accountable for regulatory compliance, quality assurance, performance management, safeguarding oversight, and the leadership and development of multidisciplinary staff teams. I was responsible for ensuring the consistent delivery of high standards of care and the achievement of positive, sustainable outcomes for young people.
